LPN, RN and LSW for Case Manager paying up to $55
SB&P Corporation is looking for motivated, independent workers for Care Management positions. We are looking for qualified candidates with one year of clinical experience that are willing to learn and have a passion for helping others. The position can be full time or part time and can perform up to 30 visits per week. These visits are to prevent hospital readmission's for patients who have been discharged and are at high risk of re-hospitalization.
